Request Medical Records

 At Doctors Medical Center of Modesto, it’s not only our job to keep you healthy. We’re also focused on keeping all of your healthcare information private. 

You can access your inpatient medical information online through our patient portal.

If you’d like a copy of your medical records, contact our Medical Records Office at (209) 576-3616. If you are picking up your medical records in person, you will need a photo identification. If somebody else is picking up your records, that person will also need a photo identification as well as a signed authorization letter from you.
